We are seeking an experienced and motivated Complex Care Practitioner to join our Continuing Healthcare (CHC) Service within North Central London. This is an exciting opportunity for a registered healthcare professional with expertise in Continuing Healthcare to play a key role in delivering high-quality assessments, reviews and case management for individuals with complex health needs.
Working as part of a multidisciplinary team, you will ensure services are delivered in line with the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care. You will undertake assessments and reviews, provide expert clinical advice, manage complex cases and contribute to the ongoing development and improvement of the service.

Main duties of the job
The successful candidate will:
- Undertake Continuing Healthcare assessments, reviews and case management for individuals with complex health needs.
- Analyse highly complex information and make evidence-based recommendations regarding eligibility for NHS Continuing Healthcare.
- Work collaboratively with patients, families, carers, health and social care professionals, provider organisations and other key stakeholders.
- Provide specialist advice and guidance on Continuing Healthcare policy, practice and decision-making.
- Chair and contribute to complex multidisciplinary meetings and case conferences.
- Support service development, quality improvement and governance initiatives.
- Manage and prioritise a varied caseload while maintaining high standards of clinical practice.
- Contribute to performance management, service improvement and risk management activities.
Support and supervise colleagues, promoting a culture of learning, development and compassionate leadership.

Essential
- Significant experience in NHS Continuing Healthcare, associated legislation and best practice
- Significant experience of successfully operating in a politically sensitive environment
- Experience of managing risks and reporting framework
- Experience of successfully managing a team.
- Demonstrated experience in a Healthcare/social care environment.
- Significant experience in working with other statutory agencies and third -party providers
- Experience of setting up and implementing internal processes and procedures
Desirable
- Previous involvement with efficiency and cost improvement programme
Skills & Knowledge
Essential
- Ability to manage aggressive/antagonistic behaviour, managing risks and resolving conflict.
- Ability to analyse very complex issues where material is conflicting and drawn from multiple sources.
- Excellent written and oral skills
- Numerate and able to understand complex issues combined with deep analytical skills
- Demonstrated capabilities to manage own workload and make informed decisions in the absence of required information, working to tight and often changing timescales
- Able to manage contentious or sensitive issues
- Ability to manage aggressive/antagonistic behaviour
- Ability to keep calm in a fast-paced environment

Education/Qualification
Essential
- Educated degree, supplemented by diploma level specialist training, experience and short courses or equivalent level of experience.
- Current registration with professional body e.g. NMC and hold a clinical qualification e.g. Registered Nurse, Registered Learning Disability Nurse or Registered Mental Health Nurse, AHP
- Membership of a professional body
