Job overview

This role involves delivering  compassionate care  to support people  to remain as independant as possible in their own homes, working as part of the Homefirst East  Team.  East Cornwall  is a large rural  area and due to the location , public transport is not available. An ability to get to visits independently is essential as is  access to your own vehicle . Travel costs for journeys during work time are supported and pool cars are also available for work use, although availability cannot be guaranteed.

You will need to demonstrate excellent  communication skills and have a positive approach to people who may have communication difficulties or who have challenging  and complex health needs.

You will be required to work a rota that covers 7 days a week  including bank holidays from 08:00 to 20:00 hours. Flexibility within this rota, and geographical location, will be important. This is a lone working  post and therefore  previous experience in a community setting is necessary.

Main duties of the job

The main purposes of the role are:

To provide a versatile and responsive service to ensure that opportunities for patients’ reablement are optimised within the community setting.

To  assist  patients to achieve their reablement goals using a range of specific activities ,without direct supervision. This may include activities for developing independence in daily living skills, supporting with personal care and promoting a return to normal function and activity with the appropriate use of aids/equipment while reinforcing self-care wherever possible.

To be  part of an Integrated Care Pathway for older patients that involve a range of services to prevent hospital admission and support discharge from hospital.

To deliver  compassionate care to patients in their own home.

To report any concerns or issues to the manager in charge and maintain accurate records using various IT systems.

To make or receive telephone calls and communicate  effectively with other professionals/ patients and their relatives  as required.

Education, Qualifications and Experience

  • NVQ Level 3 qualification in care or equivalent experience
  • Evidence of experience in care setting

Desirable

  • Additional relevant training in Healthcare
  • GCSE Maths & English or equivalent qualification
