Job summary

We are seeking motivated and compassionate individuals to join our Neighbourhood Teams across Leeds. This role offers an excellent opportunity to develop core clinical skills while delivering high-quality, patient-centred care in patients own homes and care homes.

This role will be of particular interest to current final year students on the community pathway - please indicate this clearly in your application.

We have a number of vacancies city-wide in our Neighbourhood Teams. The service currently operates between 8:00am and 8:00pm with various shifts on a 7-day rota, including weekends and bank holidays. This is a community-based, autonomous role, requiring regular travel across the locality.

A full UK driving licence and access to a car with business insurance is essential.

Main duties of the job

Working as part of an integrated multidisciplinary team, you will:

• Deliver safe, effective, person-centred nursing care to patients in their own homes with support from Senior and District Nurses.
• Undertake holistic assessment, planning, implementation and evaluation of care in line with NMC standards and Trust policies.
• Support patients with a diverse range of needs including:
• Medication administration (including insulin).
• Wound care and other clinical procedures.
• Long-term condition management.
• Palliative and end-of-life care.
• Promote health, independence and wellbeing, supporting patients to remain safely at home.
• Work collaboratively with therapy, social care and partner organisations/stakeholders.
• Recognise, respond to and escalate safeguarding concerns, ensuring the protection of vulnerable adults in line with local and national safeguarding procedures.

You will be supported through a structured development programme to build your confidence and competence, enabling you to work progressively more autonomously across an area.
