Are you a newly qualified nurse (within the last 6 months) or due to qualify soon and looking to start your career in a rewarding, supportive community setting?
We are seeking motivated and compassionate individuals to join our Neighbourhood Teams across Leeds. This role offers an excellent opportunity to develop core clinical skills while delivering high-quality, patient-centred care in patients own homes and care homes.
This role will be of particular interest to current final year students on the community pathway - please indicate this clearly in your application.
We have a number of vacancies city-wide in our Neighbourhood Teams. The service currently operates between 8:00am and 8:00pm with various shifts on a 7-day rota, including weekends and bank holidays. This is a community-based, autonomous role, requiring regular travel across the locality.
A full UK driving licence and access to a car with business insurance is essential.

Main duties of the job
Working as part of an integrated multidisciplinary team, you will:
- Deliver safe, effective, person-centred nursing care to patients in their own homes with support from Senior and District Nurses.
- Undertake holistic assessment, planning, implementation and evaluation of care in line with NMC standards and Trust policies.
- Support patients with a diverse range of needs including:
- Medication administration (including insulin).
- Wound care and other clinical procedures.
- Long-term condition management.
- Palliative and end-of-life care.
- Promote health, independence and wellbeing, supporting patients to remain safely at home.
- Work collaboratively with therapy, social care and partner organisations/stakeholders.
- Recognise, respond to and escalate safeguarding concerns, ensuring the protection of vulnerable adults in line with local and national safeguarding procedures.
You will be supported through a structured development programme to build your confidence and competence, enabling you to work progressively more autonomously across an area.
