We are offering an opportunity for a Band 5 NMC Registered Staff Nurse to join our Newcastle South Community nursing team based at Bradwell Hospital.

We are looking for a Qualified nurse who is committed to providing high standards of holistic care to our patients to join our busy, supportive & friendly team. This is an excellent opportunity for highly skilled nurse looking for a new challenge or looking to return to Community Nursing.

Many people would be unable to live at home without the support they receive from the Community Nursing teams and community services are admitting patients with a more complex acuity than ever before, with increasingly specialist treatment and care being provided by highly skilled staff. Our Community Nursing teams assess, plan and deliver nursing care to patients within their own homes, in line with evidence-based practice.

This role includes daytime working, including weekends and cover of early morning shifts starting from 7am when required.

Due to the nature of community working, ability to travel across the patch to provide care to patients is essential.

Main duties of the job

As a highly skilled NMC Registered Band 5 nurse you will work in collaboration with the muti-disciplinary team and colleagues to:

  • Promote independence and support patient centred care planning.
  • Provide care directly to patients on the caseload with a range of nursing needs (palliative & end of life care, long term condition management, medication administration, complex wound care and bladder & bowel management)
  • Work autonomously, in line with your scope of practice, competence and policies.
  • Handover patient information to the wider team, document patient information effectively.
  • Escalation of patients’ needs as appropriate.

Qualifications and training

Essential- Degree in nursing or equivalent, you are a qualified nurse with active NMC registration. Practice supervisor training or a commitment to work towards. Desirable- Community nursing experience. Genuine desire to work in a community nursing team.
