We are looking for enthusiastic staff nurse to join our district nursing team in the Tweedmouth District nursing team based in Berwick, this is a fantastic opportunity to become a team member in an exciting service committed to providing patient centred care to patients in their own homes, residential care settings and in clinics. Working closely with GP surgeries and local care homes.

Main duties of the job
Provide high-quality care and support to patients with terminal illnesses and their families, including emotional support and bereavement care.
- Undertake comprehensive patient assessments and deliver skilled nursing interventions, including wound management, palliative care, and catheter care.
- Perform and interpret diagnostic tests, including Ankle Brachial Pressure Index (ABPI) measurements using Doppler equipment, and take appropriate action based on clinical findings.
- Carry out clinical procedures requiring a high level of skill and dexterity, including venepuncture, administration of injectable medications, and removal of sutures.
- Provide patients and carers with clear information regarding treatment plans and care interventions.
- Maintain and promote high standards of professional nursing practice through evidence-based care, continuous professional development, and the sharing of knowledge and experience with colleagues.
- Work collaboratively as part of the multidisciplinary team to ensure safe, effective, and person-centred care.
- Maintain a duty of care to patients, carers, and colleagues by exercising personal and professional accountability in accordance with relevant professional standards, policies, and guidelines.
- Ensure accurate and timely documentation of patient care and maintain confidentiality at all times.
- Contribute to service improvement initiatives and support the achievement of organisational objectives and quality standards.
Detailed job description and main responsibilities
Community Staff Nurses work both autonomously and collaboratively as part of the wider multidisciplinary team. They are accountable to their line manager and the Community Nursing Team Lead, and are responsible for assessing, planning, implementing, and evaluating high-quality, person-centred care.
We manage a diverse caseload of patients with a wide range of acute, chronic, and complex health needs, delivering care in community settings while promoting independence, wellbeing, and positive health outcomes.
