This is an excellent opportunity for a Band 5 Care Home Support Nurse to join a highly motivated and dynamic team working across roles and organisational boundaries. The CHSN will provide an accessible, appropriate, high-quality, culturally sensitive and effective nursing service to support staff and residents in the care home setting, by working in partnership with service users, care providers, and other agencies. Based within Renfrewshire HSCP, the CHSN will be working alongside ANPs, CHLNs within the Care Home Support Team, GP practices with Care Homes to provide assessment and care for the patients registered within these practices. The CHSN will complement core services provided by the HSCP and will also work alongside community nursing and rehabilitation services.
