Community Staff Nurse - Care Home Nursing Support Team

This is an excellent opportunity for a Band 5 Care Home Support Nurse to join a highly motivated and dynamic team working across roles and organisational boundaries. The CHSN will provide an accessible, appropriate, high-quality, culturally sensitive and effective nursing service to support staff and residents in the care home setting, by working in partnership with service users, care providers, and other agencies. Based within Renfrewshire HSCP, the CHSN will be working alongside ANPs, CHLNs within the Care Home Support Team, GP practices with Care Homes to provide assessment and care for the patients registered within these practices. The CHSN will complement core services provided by the HSCP and will also work alongside community nursing and rehabilitation services.

Duration, Location, and Working Pattern:

• Renfrewshire HSCP
• Monday to Friday, 08:30 to 16:30 for 4 days and one day, 08:30 to 15:00

Key responsibilities:

• Deliver high-quality person-centred nursing care to residents within care home settings across NHSGGC
• Undertake comprehensive assessment planning and delivery of evidence-based nursing interventions
• Work autonomously while collaborating closely with care home staff, GPs, and the wider multidisciplinary team
• Manage and prioritise a varied caseload, including planned and unplanned care needs
• Provide clinical leadership support and supervision to junior staff, students and colleagues
• Contribute to service improvement through audit quality improvement and reflective practice
• Maintain accurate, timely clinical documentation in line with NMC and NHSGGC standards

Knowledge, training, qualifications and/or experience required to do the job:

• Registered Nurse with current NMC registration
• Significant comprehensive post registration experience within a community setting is essential
• Experience of working with adults with complex and long-term conditions, preferably in community or care home settings
• Strong assessment, clinical decision-making and problem-solving skills
• Ability to work independently and confidently while managing clinical risk
• Effective communication skills with patients, families, carers and professionals
• Evidence of ongoing professional development and commitment to revalidation
• Full driving licence is desirable to meet service needs
