Job overview

The post holder will raise the profile of smoking cessation and harm reduction  across Nottinghamshire Healthcare NHS Foundation Trust Community Mental Health Teams by providing tobacco treatment  and harm reduction advice and support to individuals and groups of people who require assistance to stop smoking or to reduce their harm from tobacco.  They will also help to prepare smokers with planned hospital admissions.

The successful candidate will be required to work flexibly across sites and develop working relationships with specialist mental health stop smoking services and community stop smoking services.

Please note that this post does not meet the pay level required for a Skilled worker visa.

Successful applicants with no prior NHS experience would normally be placed at the bottom of the band in line with Agenda for Change. This salary is below the minimum salary required for sponsorship for a Skilled Worker / Health & Care visa. In these circumstances the Trust would not, therefore, be able to sponsor for a Skilled Worker / Health & Care visa.

Main duties of the job

The post holder will provide smoking cessation support and give tobacco treatment and harm reduction advice to community patients with varying degrees of mental illness, both in clinics and in patient homes.

To Support patients who have planned hospital admissions (to both acute wards and mental health units) with temporary abstinence whilst in hospital and liaise with the inpatient smokefree teams prior to and during admission.

Use existing systems in place for identifying patients who smoke and ensure that they receive timely interventions to support them to be smokefree or to commence a harm reduction programme.

Provide advice and support to staff, reinforcing the training provided to community teams.

Refer patients who want to stop smoking or commence a harm reduction pathway to community mental health specialist stop smoking services/or community stop smoking services

Raise the profile of smoking cessation across Nottinghamshire Healthcare NHS Foundation Trust community teams.

Ensure that Smokefree documentation is kept up to date.

Please note applicants will be required to pay for their DBS check. Costs are deducted from salary following appointment. The cost of the DBS application is £26.40 (standard) or £54.40 (enhanced), this cost will be deducted from your salary over the first 2 months of employment.

You are encouraged to enrol for the DBS Update Service. An annual fee of £16 per year applies.

Provide smoking cessation and harm reduction support to community patients with varying degrees of mental illness.

Provide tobacco treatment advice for harm reduction programmes.

Support patients who smoke with planned hospital admissions (to both acute wards and mental health units).

Liaise with the inpatient Smokefree teams to identify patients who have quit smoking or who have commenced a harm reduction programme and are due to be discharged to ensure a seamless service and continuity of care.

Use existing systems in place for identifying patients who smoke and ensure that they receive timely interventions to support them to be Smokefree or to commence a harm reduction programme.

Provide advice and support to community teams.

Refer patients who want to stop smoking or commence a harm reduction pathway to community mental health specialist stop smoking services/community stop smoking services.

Raise the profile of smoking cessation across Nottinghamshire Healthcare NHS Foundation Trust community mental health units.

Ensure that Smokefree documentation is kept up to date.

Attend multidisciplinary meetings with the local mental health teams on a regular basis
