Job summary

This is a dynamic clinical and leadership role within a well-established district nursing team. The position requires flexibility to work across the Easington North locality, delivering community-based care to patients with a range of needs, including those who are particularly vulnerable and receiving end-of-life care.

The role combines direct clinical responsibilities with management and leadership duties. In addition, there is an expectation to work some core hours to attend meetings, support communication, and ensure the team remains informed of organisational developments and evolving best practice.

Main duties of the job

The successful candidate will lead and manage a defined caseload and the staff within the Easington North District Nursing Team. You will be expected to work proactively, demonstrating sound clinical judgement and the ability to make timely, risk-assessed decisions to support patients across CDDFT.

The role requires a confident, resilient, and flexible individual who can effectively manage a fast-paced and often demanding environment. Key responsibilities include conducting one-to-one supervision, completing appraisals, ensuring staff compliance with mandatory training, and identifying opportunities to develop and enhance the service in response to changing patient needs.

A significant component of the role involves Urgent Community Response (UCR). Applicants must demonstrate substantial experience working within a busy and dynamic healthcare setting, alongside excellent organisational and clinical skills.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
