Job overview

An exciting opportunity has arisen to join the Camden COPD & Home Oxygen Service as a Band 7 Physiotherapist or Nurse. We are seeking an enthusiastic clinician with experience in respiratory care gained within acute, sub-acute or community settings.

The Camden COPD & Home Oxygen Service is a community-based multidisciplinary team providing specialist respiratory care across a range of pathways, including home oxygen assessment and review, domiciliary COPD management, COPD clinics, pulmonary rehabilitation and community outreach services. The service aims to improve quality of life for people living with chronic respiratory disease, promote safe and effective oxygen use, support self-management, and reduce avoidable hospital admissions through integrated, person-centred care.

The successful candidate will work primarily within the Home Oxygen Service Assessment and Review (HOSAR) pathway, delivering specialist oxygen assessments, reviews and ongoing management for patients with complex respiratory conditions. The role also includes contributing to the wider respiratory service in line with patient demand and service priorities. Education, training and support of patients, carers and healthcare professionals form an important part of the role.

Main duties of the job

The post holder will play a key role in the delivery of the Home Oxygen Service Assessment and Review (HOSAR) pathway, providing specialist assessment, review and management of patients receiving home oxygen therapy. The caseload includes patients with COPD as well as follow-up reviews for patients prescribed oxygen for other conditions, including ILD, paediatric conditions, cluster headaches and palliative care indications. Responsibilities include undertaking oxygen assessments and reviews in clinic and domiciliary settings, performing capillary blood gas sampling and ambulatory oxygen assessments, supporting safe and evidence-based oxygen prescribing, and contributing to service development, quality improvement and stakeholder engagement.

Whilst the majority of clinical time will be spent within the home oxygen pathway, the service works flexibly and all clinicians are expected to develop competencies across the wider respiratory service to support service resilience and meet patient need. Opportunities will therefore exist to contribute to pulmonary rehabilitation, spirometry, domiciliary COPD management, COPD clinics and community outreach services.

Applicants should have a broad understanding of respiratory disease, particularly COPD, and experience managing patients with complex respiratory needs. Experience in pulmonary rehabilitation, spirometry or home oxygen assessment is desirable but not essential, as training and support will be provided.

Detailed job description and main responsibilities

  • Responsibility for the smooth running of the HOSAR pathway on a day to day basis.
  • Report to the Clinical Lead and Manager and team as required.
  • Oxygen assessment and review in clinic setting and patients home, including capillary blood gase sampling and interpretation
  • Performing all types of oxygen assessments, including ambulatory oxygen assessments
  • Organise and provide service specific in-service training (IST) and training to patients and stakeholders as required.
  • Develop and implement Camden HOS-AR protocols
  • Work with all stakeholders to promote safe and effective oxygen prescribing within guidance.
  • To communicate and present cases at the weekly team caseload and/or MDTs and support less experienced staff members with this process.
  • Collecting, analysing and reporting service data and KPIs
  • To represent Camden at relevant internal and external stakeholder meetings.
  • To ensure that high standards of clinical practice and patient care are maintained and to ensure that other clinicians in the team do likewise.
  • Supporting the delivery of pulmonary rehabilitation, spirometry, domiciliary COPD management, outreach clinics and other respiratory pathways as required.
  • Performing Clinician of the Day duties, including managing acute patient queries, responding to clinical advice requests, and reviewing all clinical referrals and communications received via the team inbox and electronic patient record systems. Responsibilities include triaging and assessing referrals, gathering relevant clinical information from referrers and GP records, determining pathway suitability, providing clinical advice where appropriate, and coordinating the management of urgent clinical issues arising within team clinics, pulmonary rehabilitation sessions and the wider respiratory service.

We are looking for an excellent communicator who can work autonomously whilst contributing positively to a multidisciplinary team. The successful candidate will be reflective, receptive to feedback and committed to continuous professional development. We value individuals who demonstrate curiosity, compassion and a growth mindset, embrace new challenges and contribute to a culture of learning, service improvement and shared decision-making.

The team is based at Peckwater Health Centre and operates Monday to Friday, 9:00am-5:00pm. The post holder will also contribute to a pro-rata senior clinician-led telephone advice service between 8:00-9:00am and 5:00-8:00pm.
