Main duties of the job

A Community Rehab Assistant will support and assist senior team members to provide care and rehabilitation in patients' own homes, aiming to prevent unnecessary hospital admissions. Responsibilities include following a care plan post-illness or hospital admission, assisting with exercise programs prescribed by the therapy team, and collaborating with other agencies. The role involves maintaining the necessary knowledge, skills, and attitudes to deliver a responsive quality service, providing clerical and administrative support, and delivering care per procedures and guidelines. You must report any changes in patient conditions to the Operational Lead. This role is full-time, requiring NVQ level 2/3 qualifications or equivalent experience. Strong communication, organization skills, and proficiency with IT systems are essential. You must drive and have access to your own vehicle for traveling across services.

Working closely with ourIntermediate Care Team, as a Community Rehab Assistant you will support and assist the senior team members toprovide care and rehab to people in their own homes.Our work is crucial to the community as we aim to prevent unnecessary hospital submission.In this role, you will care for patients in the way thatyou believe is best, delivering our high service standards, sharing bestpractice, and actively improving the way we work.

As part of our community nursing team we empower our staff to work autonomously and with flexibility by providing them with state of the artnew tabletsfor them to be able to check in from home, write their notes up on the go and allow them to not be trapped in an office base, so they can really enjoy working in their chosen community background!

This is a full-time role working 37.5 hours per week, where the role includes working early or late shifts which are 7.30-3.30 or 1-9pm covering a 7-day period. These shifts are shared equally between the team.

Within this role, you will:

  • To assist in providing rehabilitation and care in patients own home, following a care plan, designed with and for that patient, following illness or a hospital admission
  • Assist with home exercise programmes as prescribed by the therapy team.
  • Work alongside other agencies eg, Hospice Team, District Nursing Team, Adult Social Care Reablement Team to deliver joint rehab or end of life care.
  • To maintain knowledge, skills and attitudes necessary to deliver a quality service that is responsive to the changing needs of patients.
  • To provide clerical and administrative support to the team.
  • To deliver care in accordance with organisation policies, procedures and guidelines, according to competencies, as delegated by OPS Lead/ Therapist.
  • To report any changes in condition of the patient to the Operational Lead.

This is an opportunity to put your NVQ level 2/3 or equivalent/appropriate qualification/experience to great use.With strongcommunication and organisation skills, and a flexible approach will ensureyou deliver positive experiences to patients at every stage of theirhealthcare journey.

In this role, you will need to be able drive and have access in your own car, allowing you to travel across our services in a timely manner.

You will also to be confident using IT systems and be able to demonstrate working knowledge of Microsoft Office packages i.e. Outlook, Word, Excel .EMIS.
