Main duties of the job
Providing robust assessment of patients (by telephone or face-to-face) who have been referred due to presenting with mental health difficulties.
Identifying appropriate care pathways and ensuring the least restrictive option to meet the needs of the patient and ensuring their wellbeing.
Signposting to appropriate care providers (including primary and tertiary care).
Raising concerns to senior staff for support and management.
Completing documentation and planning care for patients.
Presenting cases to Multi-Disciplinary Team for discussion around care pathway.
Liaison with colleagues nursing, occupational therapy, medics, psychology and admin.
