Main duties of the job
- Assess, diagnose and treat a varied community caseload, using evidence‑based interventions tailored to individual needs.
- Collaborate across the MDT, offering expert advice and guidance to colleagues, carers and families to support safe, timely care.
- Demonstrate strong clinical reasoning, communicating clearly and compassionately—especially where there may be barriers to understanding.
- Contribute to service quality, sharing learning, supporting juniors and students, and helping to drive continuous improvement.
- Work as a proactive team member, using your initiative and prioritising effectively in a dynamic community setting.
- You are required to hold a full, valid, UK driving licence and have access to a car to use with business insurance (unless the member of staff has a disability as defined by the Equality Act 2010).

- Registered with HCPC

- Postgraduate training in community rehabilitation or related area
- Evidence of specialist CPD
Clinical Assessment & Treatment of Complex Patients

- Experience delivering physiotherapy to patients with complex needs/multi-pathologies
- Ability to undertake advanced assessment and implement evidence-based treatment plans
