Job summary

We are currently looking for motivated and enthusiastic nurse to join our Community / District Nursing Overnight Team. The role involves providing professional and holistic nursing care to patients in their own homes or care setting during the hours of 9pm until 8am the following day. The post holder will be an experienced and confident clinician with a good understanding of overnight working in the community setting. You will work alongside a well-established dedicated overnight community nursing service visiting patients throughout County Durham and Darlington. You should also have a good understanding of Urgent Community Response and enjoy a role that is dynamic and unpredictable.

Main duties of the job

This role involves providing holistic nursing care to patients in their own homes and other environments such as 24 hr care settings. A wide range of nursing skills are required for the role, including, but not limited to, venepuncture, central line care, wound care, medication administration and care for patients at end of life. Training and support can be given to gain and further develop existing skills. You will work across areas within County Durham and Darlington, visiting patients, via referrals sent to you from our Single Point of Access team. You should be a confident individual who is able to prioritise workloads and communicate concerns or issues with the wider out of hours services such as Urgent Care.

We provide hospital services from two acute sites - Darlington Memorial Hospital and University Hospital of North Durham. We have a centre for planned care in Bishop Auckland and provide care from community hospitals in Chester-le-Street, Shotley Bridge, Barnard Castle, Sedgefield and Weardale as well as over 80 other community based settings and providing care in patients' homes.
