Job summary

We are seeking a motivated and proactive Band 6 Occupational Therapist to join our integrated therapy team. You will deliver an effective, patient-centred service, supporting individuals to maximise independence through high-quality assessment and intervention.

You will manage your own caseload, prioritising workload in a fast-paced environment, and work both independently and collaboratively with Physiotherapists, Technical Instructors, and the wider multidisciplinary team.

Main Duties of the Job Undertake comprehensive functional assessments Plan and deliver individualised rehabilitation programmes Support safe discharge planning and promote independence Work closely within an integrated therapy team Maintain accurate clinical records Contribute to service improvement initiatives.

Working Pattern Service hours are Monday-Friday 8:0018:00 and weekends/bank holidays 8:0016:00. Flexible shift working is required.

Additional Requirements Full UK driving licence and access to a car is essential Role operates under Agile Working policy Person Specification (Summary) You will be resilient, organised, and adaptable, with strong communication skills and the ability to manage a varied caseload.

Main duties of the job

Working for Our Organisation Leicestershire Partnership NHS Trust works in partnership with Adult Social Care across Leicester, Leicestershire and Rutland (LLR) to deliver an integrated Home First service alongside core Community Therapy.

Significant investment in staffing ensures patients receive care from the right professional, in the right place, at the right time, enabling delivery of timely, high-quality therapy services across LLR.

Support individuals to remain safely at home during periods of health or social care crisis Facilitate timely hospital discharge with effective rehabilitation and reablement Deliver high-quality therapy interventions to promote recovery, independence and wellbeing in patients own homes We are proud of the difference our service makes.
