Job summary

Main duties of the job

  • To promote the independence and well-being of older people and vulnerable adults in line with LLR Care Home Sub Group Priorities.
  • To provide clinical oversight and advice around appropriate escalation and referral to appropriate services within CHS for identified individuals, to enable timely input and prevention of harm.
  • To work autonomously with a solution focused approach, utilising hard / soft data and resources available across the system to identify and prioritise the care homes for intervention.
  • Create data base to record interventions and to identify champions within the homes that are trained and refresher schedules.
  • To liaise with CHS, LA and ICB, and use data collection to identify key care homes for targeted intervention, and provide feedback as required.
  • To collate feedback following the training intervention and forums, to participate in evaluation and ongoing development.
  • To ensure training content is contemporaneous and in line with current standards and guidelines and update as required.
  • To maintain professional development through engagement with clinical supervision and training opportunities.
  • To work with the care home to analyse FALLS, conveyance to acute hospital settings, and referrals into Therapy.
  • To evaluate data following clinical intervention and obtain care home feedback.
