Join the Integrated Community Response Service (ICRS) as a Band 5 Community Staff Nurse and help patients remain well and safe in their own homes. You will work within a supportive multidisciplinary team focused on preventing unnecessary hospital admissions and enabling patients to return home safely following acute care. We are looking for a forward-thinking, enthusiastic, flexible and innovative nurse with a strong commitment to team working and delivering high-quality, person-centred care. You will work closely with GPs, London Ambulance Service, Social Services and other community services, with excellent opportunities to develop your community nursing skills and continue your professional development.
Main duties of the job
The post holder will assess, plan, implement and evaluate nursing care for patients in their own homes, supporting admission avoidance and safe, timely discharge from hospital. Working as part of a multidisciplinary team, they will deliver high-quality, evidence-based and person-centred care while liaising effectively with GPs, London Ambulance Service, Social Services and other community partners. The role requires excellent communication, organisation and time-management skills, the ability to work independently and use initiative, and a flexible approach to working across a 7-day service. The successful candidate will be reliable, adaptable and enthusiastic, with a strong commitment to teamwork, patient safety and ongoing professional development.
