As a community Nurse working with the Countywide night Team, most of the work allocated to you will of an unplanned nature. The service works as an on-call service covering the County of Worcestershire taking direct referrals from patients, 111, Ambulance Service and discharging hospitals. You can expect to have a comprehensive induction and work alongside peers until you are confident to then work alone. The nurses within the Countywide Nursing Team work doubled up with a Health Care Assistant, you will commence your shift at the Droitwich hub and have work allocated via a handheld tablet by the triage nurse based at Droitwich. The patients that are seen during the evening and night vary immensely and no two shifts are the same. For example, we see catheters, palliative care, wound care, administration of medication, diabetics, and patients requiring intensive support rehabilitation following discharge from hospital or admission prevention. As a team we often undertake initial assessments of patient in the community.
As part of a new initiative, we are working closely with West Midlands ambulance service and 111 and assist in responding to Urgent Community Responses. These can be patients that have fallen, or patients requiring further assessment to aid prevention in admission to hospital.
