Our team works with local GP practices (Oaktree, Rosedean and Oldbridge Surgeries) as well as health and social care colleagues, therapists and voluntary sector partners. We’re looking for someone who brings enthusiasm for community nursing, is committed to person-centred care and is keen to contribute to innovative ways of meeting the changing needs of our community. We welcome applications from nurses who are proactive, adaptable and confident in assessing and managing care in patients’ homes. An interest in working closely with patients, carers and wider MDT partners is essential, along with a commitment to continuous professional development.
The role involves regular travel to patient homes and community sites, along with the transportation of essential clinical equipment. Given the limited public transport options in Cornwall, candidates must have access to appropriate transport that can be used for work duties.

The aim is to enable them to enjoy maximum independence and optimum quality of life. To work in partnership with patients and carers to offer support and advice and promote informed choice. To work autonomously, providing effective, evidence-based care in line with competency and training record and organisational policies, procedures and guidelines. To support all locality teams by working in a flexible way. Moving to work with the team in most need on a daily, weekly or monthly basis as service provision demands.
