Job overview
We are seeking an enthusiastic and compassionate Community Midwife for Equitable Health to join our team. This is an exciting opportunity to make a real difference to the lives of women and birthing people who experience additional vulnerabilities and barriers to accessing maternity care.
Our ambition is to address health inequalities through targeted Community Midwifery support, ensuring that every woman and birthing person receives personalised, accessible and inclusive care throughout their pregnancy and postnatal journey.
Working across a variety of community settings, you will provide tailored antenatal and postnatal care to a caseload of women and birthing people with complex needs, including those seeking asylum, women in prison, teenage parents, those affected by substance misuse, and individuals with significant safeguarding concerns. You will build trusted relationships, promote engagement with maternity services, advocate for those in your care and work to improve outcomes for women, babies and families.
Working closely with safeguarding, perinatal mental health, health visiting, social care and wider multidisciplinary teams, you will play a key role in identifying risk, coordinating care and ensuring women receive the support they need at the right time. You will contribute to the development of equitable care pathways, support the implementation of the Maternity Disadvantage Tool and champion initiatives that reduce health inequalities across our communities.
Main duties of the job
- Manage a defined caseload of women and birthing people with complex social, safeguarding and health needs, including those seeking asylum, women in prison, teenage parents, those affected by substance misuse, and individuals requiring enhanced support.
- Build trusting relationships and work proactively to promote engagement with maternity services, ensuring women and families receive the right care, at the right time, from the right professionals.
- Assess, plan, implement and evaluate individualised programmes of care, coordinating support throughout pregnancy and the postnatal period to improve outcomes for women, babies and families.
- Work closely with the Named Midwife for Safeguarding, Perinatal Mental Health Team, Health Visitors, Social Workers, Obstetricians and wider multidisciplinary teams to ensure joined-up care and effective information sharing.
- Identify, manage and escalate safeguarding concerns appropriately, communicating and documenting complex and sensitive information in line with local and national guidance.
- Champion equity, inclusion and personalised care, supporting the implementation of initiatives and tools designed to address maternity disadvantage and reduce health inequalities.
- Work autonomously within local policies, national guidance and the NMC Code, using professional judgement to support women with complex and often challenging circumstances.
