We are looking for an enthusiastic, compassionate and motivated Band 6 Community Mental Health Worker to join our Adult Community Mental Health Treatment Team.
This is an exciting opportunity to become part of a multidisciplinary team that puts people at the heart of everything we do. You will work alongside service users, carers and partner neighbourhood agencies to provide high-quality, recovery-focused care to people experiencing serious and enduring mental health difficulties.
Main duties of the job
As a Key Worker, you will have a central role in supporting people on their recovery journey. You will:
- Build meaningful therapeutic relationships with service users and their carers.
- Complete comprehensive assessment, care planning and risk management.
- Coordinate and deliver evidence-based, person-centred interventions.
- Work creatively with individuals to identify their goals, strengths and aspirations.
- Work collaboratively across the multidisciplinary team and wider partner agencies to ensure care is coordinated and effective.
- Provide professional advice, guidance and support to colleagues and external agencies.
- Use your experience and clinical expertise to support, coach and develop junior and less experienced staff.
- Act as a positive clinical role model, contributing to a culture of learning, innovation and high-quality care.
