Blackpool Community Mental Health Team is seeking to appoint a Band 5 Community Mental Health Nurse/Social Worker.

This role will be responsible for delivering holistic care to patients in a community setting including homes and clinics. Therefore you must be able to manage the travel requirements of the role to deliver a comprehensive service.

Main duties of the job

We are seeking a motivated and compassionate band 5 community nurse or Social Worker to join our dynamic community healthcare team. You will provide high quality patient cented care – promote independance and support patients with a wide range of health needs
