An exciting opportunity has arisen for a Community Mental Health Nurse to join our Memory Team in Knaresborough, based at Alexander House. We are looking for a dynamic, positive and forward-thinking clinician with a passion for caring for people with cognitive impairment, their families and carers, and with a specialist interest in delivering high quality, effective and place-based Memory Services.

This is an exciting time to join our team and locality - this role is an important part of a quality improvement initiative, alongside the trust Dementia pathway refresh. We deliver our memory services to broaden access and ensure delivery of specialist memory assessment in a timely manner. This is essential to enable timely diagnosis of dementia and associated needs and conditions, which supports access to early treatment and support, decision making and aids optimal management and life outcomes for those in need.

Main duties of the job

The ideal candidate will be highly motivated and be able to demonstrate best practice in community mental health service delivery for older people, their families and carers. The ability to work independently, effectively and collaboratively within multidisciplinary teams, with external partners, and most importantly - with people, their families and unpaid or paid carers, is an essential core competency.

In the role you will be responsible for making the initial contact to support service users and their families and carers in their understanding of the memory pathway. You will undertake a detailed history using a holistic memory assessment framework, with the initial collation of all clinically relevant individual service user information (clinical history) and involving families and carers as part of our genuine commitment to co-creation. In this way you will promote positive experience and clinical outcomes from the beginning of the person's service journey.

There is a requirement to accurately record and document clinical discussions and assessment details on our clinical system and to ensure that all individual service user documentation is up to date and available to inform internal clinical meetings and aid consistent, safe and high quality decision making by members of the MDT and wider colleagues

Harrogate Memory Team is a small but lively, dynamic, and supportive team committed to collaboration and joint working. You will support the team's improvement activities for co-creation, aiming to deliver care that is highly personalised to each individual's needs and working closely with families, paid and unpaid carers. You will be encouraged and supported to bring your skills and experience to contribute to our team and locality's service development and continuous improvement and innovation endeavours.

Alongside the commitment from the Trust our team is small and friendly with a genuine sense of the importance of promoting staff well being. The Team has an identified and proactive well being champion, who aims to provide a range of opportunities for the team to think about how they stay well at work and in their personal lives.

The team is based in the same office space and works closely with the Harrogate Community Mental Health Team for older people, so you will benefit from extensive MDT support and input in your role.

You will have an enthusiastic approach towards your own personal and professional development, being open-minded to changes in the way that the pathway is delivered in order to support service improvement and effectiveness of delivery so that we can improve experiences of care.

In return, you will receive regular appraisal, clinical, management and caseload supervision and will benefit from the support of an experienced and committed multi-disciplinary leadership team, including the Team Manager and the Clinical Specialist Nurse/Advanced Nurse Practitioner.

Due to the nature of the role it is essential that applicants are able to travel independently across the locality and have access to a suitable vehicle for business purposes. If necessary, adjustments can be considered in accordance with the Equality Act 2010.
