ACCOUNTABLE TO Head of Community Disability Services

JOB SUMMARY

The post holder will provide a community nursing service to adults with a Learning Disability and will work within the multi-disciplinary team, offering specialist nursing intervention, support, knowledge and advice. They will be responsible and accountable for the delivery of an accessible, appropriate, high quality and effective nursing service under the direction of the Team Leader and Community Learning Disability Nurse Lead. They will promote a person centred approach to care and carry continued responsibility for the assessment of nursing needs, risk assessment and the development, implementation and evaluation of nursing care plans by working in partnership with service users, carers, case managers, providers and other agencies.

KEY DUTIES / RESPONSIBILITIES

Assess, plan, implement, evaluate and review holistic, person-centred nursing care for service users with a wide range of acute and long-term conditions and complex care needs e.g. mental health, dementia, forensics, autism and/or behaviours, which challenge.

Management and monitoring of physical health conditions e.g. diabetes, epilepsy, working in partnership with specialist services, primary care services, statutory and independent care homes, supported living and day care services

Undertake relevant clinical/therapeutic activities, nursing procedures and prescribed treatments following appropriate training and assessment in a safe and professional manner. E.g. venepuncture; enteral feeding; catheterisation: depot injections; medication monitoring

Undertake evidenced based nursing assessments e.g. continence, MUST, Braden, NEWS2 and work in collaboration with multi-disciplinary team colleagues to complete specialised assessments where appropriate

Co-produce nursing care plans with service users and their carer’s.

Develop therapeutic relationships that support informed decision making, self-management and promote independence, ensuring ongoing evaluation of care to enable an understanding of the nature of the care, treatment and progress.

Undertake a wide range of person-centred risk assessments and formulate risk management plans to the required level dependent on the risks identified relevant to the service users assessed needs e.g. Division’s Care and Risk Management Plan or Promoting Quality Care: Good Practice Guidance on the Assessment and Management of Risk (2009) as required.

Monitor and report on service user progress and maintain accurate, up to date and detailed contemporaneous records in line with Trust Policies and procedures, in keeping with the NMC Code.

Uses a range of clinical assessment skills including- history taking and physical assessment to inform clinical decision making and onward timely referral as appropriate to other teams/services, based on the assessed needs of the person.

Adhere to current policies and procedures in relation to the ordering, storage and administration of medicines and controlled drugs and transcribing.

Ensure the provision of an integrated quality service by maintaining effective communication with the multi-disciplinary team and community services providing a range of services to adults with a learning disability and their carer’s.

Undertake Carer assessments as required through use of the Carers Conversation Wheel / NISAT Carers Assessment of needs.

Promotion of health and wellbeing, and implement health promotion initiatives in conjunction with Specialist services and primary care services.

Participate in the review process with service users and their carer’s as appropriate in line with the Trust case management arrangements and other statutory requirements.

Assist in the monitoring and review of agreed packages of care appropriate to service users’ needs in partnership with the case manager.

Monitor and review the standards of care implemented in statutory and independent care homes, supported living and day care services undertaking audit as required.

Provide nursing support and interventions including the delivery of training and completion of competency-based assessments with staff employed in day centres and supported living facilities, in accordance with agreed protocols.

Work collaboratively as part of the multi-disciplinary team so that service users can be cared for safely in a variety of community settings reducing unnecessary hospital attendances or admissions and facilitate timely discharge from hospital.

Participate in Adult Safeguarding procedures as directed by the Team Leader, undertaking and completing Investigating Officer training (IO) following appropriate training.

Ensure the implementation of the Mental Capacity Act (Northern Ireland) 2016 where appropriate and the completion of relevant capacity assessments as required following appropriate training and experience.

Undertake office duty on a shared rota basis as required.

Observe and monitor the effectiveness and use of aids or equipment within the service users home in conjunction with the occupational therapist.

Maintain own professional and personal development and the maintenance of professional competence in accordance with the NMC (2018) The Code: Professional Standards of Practice and Behaviour for Nurses and Midwives and NMC revalidation and contribute to continuous quality improvement.

Ensure practice reflects compliance with the standards set in the NMC Code of Professional Conduct and clinical policies, guidelines and protocols, and complete tasks necessary to maintain NMC registration.

Delegate safely in accordance with NMC Code and NIPEC Delegation Framework (2019).

Participate in the development of clinical pathways, evidence based practice and an outcomes based approach to safe service delivery.

Participate in and facilitate in the training and induction of nursing staff including the supervision of clinical practice and preceptorship for newly qualified Band 5 nursing staff to facilitate the delivery of optimum high quality care.

Participate and contribute to own professional and operational supervision.

Provide professional nursing supervision and undertake revalidation with Band 5 nursing staff within the team

Facilitate the teaching and assessing of nursing students and meet the requirements of the NMC Education standards, fulfil the role of practice supervisor, and practice assessor.

Participate in annual staff appraisal / development review, ensuring effective implementation of the Knowledge and Skills Framework (DH, 2004).

Ensure near misses, incidents, accidents and faulty devices are recorded, reported, investigated and learning disseminated as per HSC Trust policy to facilitate the delivery of safe, effective, person centred care and services.

Ensure effective systems are in place to gain service use and carers feedback on their experience of care.

Contribute to service delivery ensuring service users, carers and other stakeholders are consulted and identify gaps in service provision.

Work to deadlines, organise self and manage time appropriately.

Implement and contribute to the Trust’s Clinical and Social Care Governance Framework.

To undertake any other duties defined by the Team leader and Community Learning Disability Nurse lead to meet the changing needs of the service.
