To provide support and guidance to the specialist Learning disabilities Multi-disciplinary Team in Walsall
To support the Walsall people, their carers and family through assessment, nursing intervention, and care planning.
Main duties of the job
To promote a positive approach to health education and health promotion. Participate in health education, sexual health education and healthy lifestyles.
Monitoring and managing their own clinical caseload (Band 6)
To maintain, as required, record keeping within NMC and Trust Guidelines
To provide clinical activity information within the time frames required
To comply with the Health and Safety at Work and fire regulations in the work place.
To participate in own appraisal and ensure objectives and work plans are met.
To comply with all Mandatory training requirements.
To participate in audit and service evaluation programmes as required
To undertake any other duties of a similar nature consistent with the responsibilities of this post in order to provide a quality Service.
To maintain up-to-date knowledge of development within the sphere of learning disabilities
To participate in team meetings and to contribute to team and service development
To ensure knowledge of safeguarding Vulnerable Adult and take action to safeguard children, young people and adults where necessary.
Develop effective working relationships with professionals both internal and external to the service
To ensure knowledge and understanding of the Mental Health Act
To co-ordinate care for citizens who are subject to s117 aftercare
To participate in own appraisal and ensure objectives and work plans are met.

Detailed job description and main responsibilities
To work pro-actively with all members of the community nursing team and the wider multi-disciplinary team.
To participate in specialist assessments; formulation; implementation and evaluation of measurable interventions
To contribute to risk assessments and detailed health interventions
To be a role model demonstrating high standards of care
To maintain clear and contemporaneous records of all case work, using approved electronic recording systems
To effectively communicate with a range of stakeholders across a range of settings.
To work within the guidance of the Trust/local ‘Lone Working’ policy, using agreed ‘report & confirm’ systems.
To Carry a caseload under the supervision of the line Manger
To be organised and able to manage own time
To actively participate in regular clinical supervision with the community nurse manager.
