We have a vacancy to join our team to continue strengthening community support for adults with Learning Disabilities, in the east community team in Cornwall. This is a band 5 community nurse post. The post will join an established nursing and MDT team within the west of Cornwall. If you are newly qualified, the trust offer an established preceptorship programme to support you at the start of your career. You will be responsible for undertaking nursing duties in line with agreed Care Processes and Pathways. You will work actively with the individual and their families and/or carers providing person-centred, skilled and effective assessment, intervention, monitoring and review. You will be responsible for delivering evidence-based practice, within an innovative, multi-disciplinary team with a robust supervision and appraisal culture. There are CPD, research and audit opportunities within the team and wider trust. There are good levels of service user engagement and joint working with other teams including Acute and Primary Care liaison, Social Care, Mental Health, Forensic and Children’s, etc Clinical work is multi-disciplinary involving assessment of complex cases, formulation and provision of therapeutic interventions, as part of the multi-disciplinary care planning and delivery. Planning, directing and supervising associated work for HCAs and delivery of patient-centred training packages to care providers.
The fixed term contract end date will be 30/04/2026.
As part of the Community Learning Disability Team you will provide person-centred, skilled and effective assessment, intervention, monitoring and review for designated individuals who are eligible for the Learning Disability Service. You will be responsible for the maintenance of evidence-based learning disability nursing practice, contributing to and facilitating Care Processes and Pathways. You will manage and supervise the work of others, and act as a mentor and preceptor.
