Job summary

We have an exciting opportunity for a Community Nurse in our Kirklees Learning Disability Health Team.

The Team consists of a comprehensive multi-disciplinary team of psychiatrists, psychologists, learning disability nurses, occupational therapist, physiotherapist, speech & language therapists, dietician and healthcare support.

The successful candidate will have a good understanding of the complex needs of people with learning disabilities and demonstrate the qualities to deliver safe, effective and personalised care to meet the needs of service users. You will be committed to improving outcomes for our service users and have excellent interpersonal and communication skills. You will need expertise and skills to identify health needs and support required, as well as providing health promotion to empower people our service users.

Main duties of the job

The successful candidate will work with people with learning disabilities in a variety of community settings to provide a high quality and dynamic specialist nursing support with a clear focus on maintaining and improving the health and wellbeing of adults with learning disabilities.

The role will involve working with service users, families, carers and other members of their support networks in a holistic way keeping the person at the centre of their care.

The post holder will also support and educate other health providers in making reasonable adjustments and facilitating bespoke care pathways to care to ensure that people with a learning disability in Kirklees have the same access to health care. The role will also include promoting the service to external providers and taking part in health promotion activities.
