
An exciting opportunity for a band 6 Junior Sister, Community Nurse, has arisen in the Sudbury Integrated Neighbourhood Team. The team are based in a modern Health Centre in Sudbury which is a historic market town surrounded by picturesque villages. The Sudbury Health Centre is the base for a wide range of other health professionals and agencies including Adult Social Care and a GP practice so we are able to work together in a fully integrated way.
The successful applicant will join a dynamic, friendly team of community nurses, community matrons, therapists and social care staff. You will be providing nursing services to patients in the community and have the chance to work in an autonomous manner with the support of the wider multi disciplinary team within the hospital and across the West Suffolk Alliance. Within your role you will be able to provide clinical and supervisory leadership to staff and students within the team and support the management of the nursing staff by taking responsibility, in partnership with the District Nurse, for the day-to-day co-ordinating, planning and delivery of the nursing services within the Community Healthcare Team. You will be supporting the management of patients with multiple and complex needs, using evidence based practise to assess, plan, implement and evaluate interventions to improve patient health.
On site at the health centre you have access to a range of treatment facilities and may be involved in delivering nursing care in specialist clinics. You will be using a wide range of nursing skills and will need to be confident in your assessment skills and in your ability to provide safe, effective treatments whilst working in a community setting. Experience in leadership as well as clinical experience in palliative care, wound management, catheter management , cannulation, IVs and a range of point of care testing techniques will be an advantage. You will have the chance to be fully involved in working with the MDT and integrating with Adult Social Care, hospice and hospital staff and will need to be committed to ongoing learning and development in the team. You will need to be a car owner and driver. We operate a 7 day service within core hours 8am to 6pm
