Job overview

We are seeking to appoint an enthusiastic, motivated and reliable Community Health Care Support Worker, who is keen to deliver a range of quality care services to patients visited by the Community Health Support Team based at Civic Way, Ellesmere Port. The team visits patients and their carers of all the Integrated Community Care Teams, across Western Cheshire.

The hours are 36 per week. Consisting of  4 x 9 hour shifts, worked on a rota basis between 22.00 – 07.00 including Bank holidays and weekends.

The Trust currently supports the Real Living Wage (RLW) hourly rate as set by the Living Wage Federation and will apply a pay supplement if the national A4C hourly rate falls below this.

Main duties of the job

As part of a friendly team, you should have experience of working in patients' homes and the ability to carry out tasks such as simple dressings, catheter care, personal care, recording blood sugars and have experience of providing care for palliative patients. This post involves Lone working providing overnight care in patients homes.  This post requires travel across the Western Cheshire footprint, access to transport for work purposes is essential and MUST be demonstrated.
