Job overview

Cardiff West Cluster comprises seven GP practices working collaboratively with partner organisations to support the health and wellbeing of our local population. We are seeking a motivated and proactive Pharmacy Technician to join our multidisciplinary team and support the delivery of high-quality medicines optimisation services across the Cluster.

Working closely with Cluster Pharmacists, GP practices, community pharmacies and other healthcare professionals, the postholder will play a key role in supporting patients following discharge from hospital. A primary focus of the role will be processing discharge advice letters, undertaking medicines reconciliation, updating prescribing records and helping to ensure safe continuity of care between primary and secondary care services.

This is an exciting opportunity to contribute to improving prescribing safety, enhancing patient outcomes and supporting the effective use of medicines across the Cardiff West Cluster. The role will involve working across GP practices within the Cluster and contributing to a range of medicines optimisation initiatives.

Main duties of the job

• Process hospital discharge advice letters and undertake medicines reconciliation following discharge from hospital.
• Work closely with Cluster Pharmacists, GPs, practice teams, community pharmacies and other healthcare professionals to support safe transitions of care.
• Update and maintain accurate patient medication records and prescribing information using clinical systems.
• Support safe and effective repeat prescribing processes and medicines optimisation initiatives.
• Assist in identifying and resolving medicines-related queries to improve prescribing safety and patient outcomes.
• Contribute to audit, data quality and service improvement activities across the Cluster.
• Support the reduction of medicines waste and promote the safe and effective use of medicines.

This is a fixed-term 18-month post from start date funded through approved Cardiff West Cluster funding.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Detailed job description and main responsibilities

The successful candidate will support the delivery of medicines optimisation services across the Cardiff West Cluster, with a particular focus on medicines reconciliation following hospital discharge and ensuring accurate, up-to-date prescribing records.

Working as part of the multidisciplinary team, the postholder will collaborate closely with Cluster Pharmacists, GP practices, community pharmacies and other healthcare professionals to support prescribing safety, continuity of care and the effective management of medicines. Responsibilities will include processing discharge communications, implementing medication changes, supporting repeat prescribing processes, maintaining clinical records and contributing to audit and quality improvement activities.

The postholder will work across GP practices within the Cardiff West Cluster and will play an important role in improving patient safety, reducing medicines-related risks and supporting the delivery of high-quality primary care services.
