Clinical Triage PractitionerAn exciting opportunity has arisen for a Clinical Triage Practitioner to support the delivery of Children's Specialist Services across Bath & North East Somerset, Swindon and Wiltshire. Main duties of the job

You will support the Clinical Triage Lead in the delivery of multidisciplinary triage for complex and multiple-need referrals. You are also responsible for providing specialist clinical advice and support to Care Coordination and Single Point of Access functions and ensuring children, young people and families are directed to appropriate services through effective care navigation. In this role, you will communicate complex, sensitive and potentially distressing information to families and professionals as required and work collaboratively with health, education, social care, and third-sector organisations to ensure coordinated care delivery. Additionally, you will identify, escalate and manage safeguarding concerns, clinical risks, and pathway issues while contributing to service development, governance processes, and the implementation of standard operating procedures.

Clinical Triage Practitioner
