An opportunity has arisen to offer a 12 month Fixed term Band 3 role within our CYPF Intensive Support community team supporting young persons with a learning disability and or Autism and their families who come in to contact services at difficult times in their lives
Main duties of the job
To provide a high standard of person-centred care under the supervision of nursing staff and allied health professionals, to young people with learning disability diagnosis who may have complex needs and/or behaviours that challenge.
To support carers and families in maintaining a safe care environment in line with Positive behaviour support approach. To reduce the risk of placement break down and admission to hospital setting.
Detailed job description and main responsibilities
To assist in assessment of the client as support to and under the supervision of, a registered nurse or allied health professional staff. • To follow individualised care and therapeutic programmes for patients under the instruction of the nursing staff and allied health professionals. • To be aware of the client’s clinical and current presentation and individual circumstances to ensure the care and comfort of clients. • To contribute to the clients plan of treatment by communicating and documenting any observations of patients behaviours and/or physical health concerns to the nursing staff/team/MDT. • To promote communication and act as an advocate for patients who are experiencing difficulty in communicating. • To work with all members of the multi-disciplinary team to promote quality care. • Work in collaboration with the client, involving them at all times with their care. • To ensure, by attitude, approach and response, the continuation of good working relationships between staff, clients, families and care providers. • To support the team in the referral, assessment, intervention and discharge stages.
