Job overview
This role is patient facing, supporting the staff, patients and multi-disciplinary team who are within the hospital. The Practitioner role is very clinically and quality focussed and will be responsible and accountable for ensuring standards of care are maintained and improved overnight, at weekend and bank holidays, in conjunction with the CSM who will support team allocation. Experience of managing complex patient groups such as end-of-life patients, dementia, delirium, infection prevention and those at risk of harm from pressure tissue damage. The CSP will possess a high level of clinical skills and have the ability to develop staff working during the out-of-hours periods to develop their own skill sets.
The Clinical Site Practitioner will support improvements in patient safety and patient experience as well as evidenced based nursing practices, leading by example, motivating and empowering other staff to use strategies to provide the best care. Clinical Site Practitioners are highly visible and accessible for patients, visitors, and staff and communicate with the operational teams to promote safe patient flow, where possible, within the hospital overnight, weekends and bank holidays ensuring patients are cared for in the right environments. Close working relationships with the Site Operations Team, Critical Care
Outreach Team and specialty medical teams will ensure a seamless and resilient service is provided to patient, staff and visitors at all times.

- High levels of practical support is given to ward teams
- Quality indicators are achieved including falls, pressure ulcers, sepsis etc.
- Practices are quality driven in all areas
- High standards of care are set and delivered
- Patients are treated with compassion, dignity and respect, placing value on diversity
- Patients are cared for in a clean environment
- Patients are managed in line with practices that support the deteriorating patient
- Best practice in infection prevention control is practised
- Monitor and drive CQC fundamental standards and compliance
