Job overview

The postholder will lead the day-to-day operational delivery of the Patient Flow and Discharge, deputising for the Deputy Director of Nursing and AHPs when appropriate.

You will be responsible for ensuring our patients receive safe, efficient, high-quality, and cost-effective care across all their areas of responsibility. You will also be expected to take an active role in the Trust’s corporate objectives leading on some Trust wide initiatives/developments, working with system partners to support delivery of safe and timely discharge.

The postholder will ensure that services are designed to meet the evolving needs of the population in line with current government initiatives, Trust, and local targets.

The postholder will be a registrant and will act as Clinical Lead for the service.

The postholder will have several years of experience working at Senior Nursing, AHP, ODP, HCS level and will be able to use their clinical expertise when dealing with stakeholders. You will lead on the development, implementation, and evaluation of the clinical elements of service redesign, acting as an expert resource to key stakeholders, managerial staff, patients, and carers.

The postholder will be the clinical expert providing a consistently high level of service within the resources provided.

Main duties of the job

  • The post holder will work closely with teams across the organisation to support planning and delivery of exemplary services. This will include ensuring robust governance, and that processes and procedures are always followed. They will support the divisional SLT in providing clinical and operational leadership and management across the services. Take responsibility for day-to-day delivery of services within the Directorate, ensuring operational performance is safe, effective, and patient-centred.
  • Hold line management responsibility for the services within the Care Group, Patient Flow and Discharge team, ensuring that personal development needs are met.
  • Ensure that the Patient Flow Team provide: 24/7 safe patient placement; Appropriate management of inpatient bed allocation; Professional and clinical leadership for all clinical areas (in and out of hours); Lead the operational out of hours response to any critical or major incidents.
  • Ensure that the discharge team provide:   Nursing leadership to the management of safe patient discharge form the hospital;  Expert knowledge of managing complex patient discharge patient pathways;  Ability to communicate effectively and work collaboratively with partner organisations across the system; A point of contact for staff, patients, relative and other organisations in managing patient discharge.
