Main duties of the job

As our new Clinical Reconnect Practitioner, you will:

  • As a Clinical Reconnect Practitioner you will work autonomously in prison, community and police detention cells, to identify and respond to requests of screening and assessment of people with identified health needs leaving the prison establishment within a 12-week period or less.
  • Take referrals for service users who are leaving the prison establishment within 12 weeks left to serve or up to 28 days post release. The aim of the service is to reduce reoffending by addressing the health and social needs of the service user and ensuring onward referrals are made to ensure continuity of care provision
  • Using validated tools to assess for several identified health needs and utilise information accordingly to inform decision-making and facilitate diversion within and out of the criminal justice pathway, creating a clear individualised support plan. Including preparing accurate written reports following assessment in line with quality standard

Qualifications and skills required:

  • Qualified Registered Nurse, Mental Health Nurse, Social Worker or Occupational Therapist with current registration with the relevant professional body
  • Substantial evidence of continuing professional development and post registration training.
  • Experience of working with patients, face to face, who have complex mental health conditions
  • Awareness of psychosocial interventions available to service users and how to refer into service
  • Awareness of substance misuse services and support available
