Job summary

The Colne Practice, located in Rickmansworth, is seeking a qualified clinical pharmacist to join the team for three days per week. Ideal working days will be Monday to Wednesday (negotiable) working 9am to 5pm.

Main duties of the job

  • Clinically assessing and treating patients
  • Providing clinical advice and expertise on medication
  • Providing specialist expertise in the use of medicines and medicine information (e.g. side-effects, dosage)
  • Providing pharmaceutical medication review and health education advice to patients, by face-to-face, telephone or notes-based review, as agreed with the practice
  • Ability to plan, manage, monitor, advise and review general care plans for patients with long-term conditions
  • Contributing to public health campaigns, including flu vaccinations, childhood immunisation and adult immunisation programmes
  • Processing specialist treatment and hospital medication discharge letters
  • Monitoring patients with respiratory conditions
  • Establishing ongoing professional relationships with individual patients

About us

The Colne Practice is a friendly GP practice with a list size of approximately 10,500 patients. Our team comprises 7 GP partners, 2 practice nurses, 1 HCA, 2 clinical pharmacists and reception and admin teams. We have on site staff parking.
