Main duties of the job

This is a dynamic, patient-facing Clinical Pharmacist role, focused on delivering evidence-based pharmaceutical care in Primary Care. You will use your expert knowledge of medicines to support patients with specific disease areas and long-term conditions.

Key responsibilities include:

  • Clinically assessing, diagnosing, and prescribing (where qualified)
  • Optimising medicines and improving prescribing safety
  • Collaborating with a multidisciplinary team to enhance patient outcomes
  • Supporting medication reviews and structured medicines optimisation
  • Contributing to service development and innovative working practices
  • Working with Divisional Boards and the National Modality Business Support Team to deliver strategic priorities
