Job overview

This exciting role will provide and be part of the clinical pharmacy service to various sites across Cornwall. This role will include, attendance at ward rounds, support self-medication, education of members of the multi-disciplinary team and patient/carer counselling. You will be supporting the lead pharmacists in the Initial Response Service (IRS) as part of the Mental Health service and within the Adult Community hospital setting to review and provide clinical pharmaceutical input and care to patients and those transferred from an acute trusts. You will have the opportunity to develop managerial skills and support the wider pharmacy team including part of a 7 day service rotation.

We welcome applications from individuals who are motivated to optimise patient care and develop pharmacy services. You will be supported with both the NMP and the ACP pathway. We will consider previous experience and applicants from any sector. For those with limited experience but the right passion and drive we would consider a band 6 to 7 progressional post. The role is a full-time post but alternatives will be considered.

Main duties of the job

The pharmacist, with the support of the lead pharmacists, will provide highly specialist, advanced clinical and professional pharmaceutical services to patients. This role is split role working with patient who come in contact with the Initial Response Service (IRS) and receiving Crisis service mental health support and also supporting patients who require support from the Community Hospitals under the care of Cornwall Partnerships Foundation Trust to promote the optimised, safe and cost-effective use of medicines You will be a core member of the Trust’s urgent care team, providing professional pharmaceutical governance, medicines safety, medicines information, medicines finance, education and training and medicines optimisation support to the team and the service’s patients. You will build relationships and work with internal stakeholders and external partners to support the delivery of high-quality pharmaceutical care to all patients
