Job overview

The appointee will join the existing multi-disciplinary respiratory team, and work in conjunction with their team members in coordinating and developing the service. The post holder will work collaboratively with healthcare professionals and stakeholders involved in the provision of integrated care, to ensure joined up care for patients across acute, community and primary and secondary care settings.

This post would suit a Nurse with a sound respiratory knowledge base who is interested in contributing to the delivery of Integrated Respiratory Care in those with chronic respiratory conditions. The post holder will have a good working knowledge in the diagnosis and management of Asthma and COPD, confident clinical assessment skills and have experience working as an autonomous practitioner.

The service has been running since October 2016 and continues to grow and develop. The Hillingdon Integrated Respiratory Service (HIRS) provides consultant and nurse led community clinics, early supported discharge services and admission avoidance services across the borough of Hillingdon. Pulmonary rehabilitation and home oxygen services are provided within Hillingdon by Harefield Hospital and there are good links between the two organisations.

The HIRS participates in the National COPD Audit and has proudly achieved the best practice tariff for the last financial year and have been highlighted as a positive outlier nationally.
