The post-holder will work closely and collaboratively with all members of the multidisciplinary team to provide exemplary care to patients with FH, dyslipidaemias and complex cardiovascular needs. We serve a diverse cohort of patients and their families. The post-holder will work proactively to deliver safe and effective specialist nursing care, as well as contributing to the improvement of the service and the development of this exciting speciality. Applicants must have excellent communication, organisational and time management skills. As one of the country’s leading specialist cardiac centres, aiming to be at the forefront of care provision and service design, it is expected that the post-holder will work closely with both regional and national CNSs from other Trusts, national FH and CVD prevention professional organisations and third-sector colleagues to provide comprehensive and holistic care for our patients. The successful candidate will embody the Trust’s WeCare values and these will shape their everyday working.
Main duties of the job
The aim of the Familial Hypercholesterolaemia (FH) Clinical Nurse Specialist (CNS) service is to provide expert, specialist nursing care to patients living with FH. The post holder will provide clinical support to this large, and growing, patient cohort. By participating in CNS-led clinics, contributing to multidisciplinary clinical review meetings and working collaboratively with the interprofessional team across primary, secondary and tertiary health care settings, the successful candidate will contribute to providing gold-standard, joined up care to this group of patients.
In addition to direct patient care, the successful candidate will contribute to service development by participating in and leading service audit and QI projects. By participating in and leading research, publication and education, the FH CNS will also contribute to the advancement of the field of FH nursing, both locally and nationally. They will work collaboratively with relevant third sector organisations to ensure holistic patient education, support and empowerment across their healthcare journey. The successful candidate will be committed to life-long learning and will be supported to undertake formal learning and independent professional development.
