An exciting opportunity has arisen for an experienced and enthusiastic pharmacist to join our friendly and forward-thinking team at Chesterfield Royal Hospital NHS Foundation Trust. We are seeking a pharmacist who is highly motivated, innovative and dynamic to lead the pharmacy clinical services within Respiratory medicine.

Pharmacological management of respiratory diseases is a continually evolving field and this is the perfect opportunity for the post holder to develop and gain experience within an established advanced practice role, supported by the respiratory MDT to improve medicines safety within this specialty.

Main duties of the job

You will undertake a lead pharmacist role working closely with the respiratory multidisciplinary team. You will provide the pharmacy clinical service to the respiratory ward but also develop your own portfolio of advanced practice.

Working within the multidisciplinary team you will provide pharmacist led asthma and ILD clinics, where you will initiate treatments with associated counselling review of relevant laboratory monitoring, and fulfil the relevant commissioning criteria through the blueteq system. You will support the provision of respiratory biologic medicines via homecare services and be responsible for repeat prescribing of these medicines.

You will lead respiratory biosimilar switches by supporting the clinical teams and through direct patient contact

You will provide clinical supervision and mentoring to pharmacists, nurses and medical staff within the team and proactively contribute to the wider medicines safety agenda

Detailed job description and main responsibilities

Respiratory Specialist Role:

• To lead the provision and development of clinical Pharmacy services to respiratory medicine.
• Provide support and oversight of specialist medicines use within respiratory medicine
• Contribute to eligibility assessment process before initiation of new therapies
• Manage the addition of patients to BlueTeq as necessary
• Arrange homecare for appropriate patients and provide ongoing support as needed
• Provide counselling for patients on new biologic/ specialist medicines
• Undertake prescribing and coordinate repeat prescribing for biologic / specialist medicines
• Contribute to the annual clinical review of patients on biologics
• Support biosimilar uptake and switches as needed
• Contribute to ILD clinic in managing high risk immunosuppressants and steroids
• Provide support for repeat prescribing of TB medicines
• Contribute to asthma MDT and provide specialist advice on medicines optimisation issues
• Lead on sustainability (greener) agenda for medicines used within respiratory medicine
• Undertake clinical reviews and support for patients on high dose or weaning courses of corticosteroid medicines
• Take a lead on issues relating to prescribing and use of nicotine replacement therapy in line with trust smoking cessation agenda
• Lead the review of specialty requests for new medicines. Including formulary management and new product requests for D&TC. Provision of expert advice and information by evaluating and interpreting published evidence.
• Facilitate the development and implementation of medicines related guidelines
• Support quality improvement and audit relating to medicines used within respiratory medicine

Ward Based Clinical Practice:

• Discharge
• Take a proactive approach to the management of ward discharges, to include prescribing of discharge medicines in line with local guidance
• Work collaboratively with medical and nursing teams and Trust Operations Team to facilitate and expedite discharge
• Ensure high quality patient counselling is delivered by the MDT during admission and at discharge
• Support communication to GPs, community pharmacy and other primary care services
• Medicines optimisation
• Undertake medicine optimisation activities in line with clinical specialty
• Provide clinical pharmacy support to consultant ward rounds, MDT meetings and ward operational meetings
• Work with prescribers and nursing teams to ensure high standards of medicines use including development and review of relevant guidelines
• Review & investigate medicines incidents and liaise with the ward team to adopt changes in practice where necessary
• Take a lead role in managing ward based medicines expenditure, to include:
• Ensuring multidisciplinary ward team take a thorough approach to obtaining patients own medicines from home
• Ensure the ordering and supply of medicines receives appropriate scrutiny
• Provide and present reports to the MDT, ward team and pharmacy in relation to medicines expenditure
• Contribute to the pharmacy clinical economy group agenda

Governance Role:

• Attend appropriate Care unit / specialty meetings to ensure that pharmaceutical issues are considered.
• Lead the review of medicines related incidents within the speciality
• Monitor interventions in treatment to identify training and information need
• Provide training to Care unit / speciality staff
• To monitor compliance and advise on the implementation of National Guidance e.g. NICE

Training and education

• To provide training to pre-registration students and pharmacists, including tutoring for the MSc/Diploma in clinical pharmacy
• To provide training and education to other healthcare professionals
• To provide professional supervision to technical and supporting staff and to undertake the role of Responsible Pharmacist when required
• To contribute to the delivery and development of clinical pharmacy services across the Trust

GENERAL RESPONSIBILITIES

• To undertake any training (managerial and professional) identified by staff appraisal
• To participate in weekend and Bank Holiday working in accordance with agreed rotas and needs of the department
• In the event of a disruptive incident, all Trust employees may be required to attend work if they are fit and well and able to do so in line with business continuity and major incident plan
• To maintain confidentiality at all times relating to patients, staff and the Trust
• To actively participate in the Staff Appraisal Process
• To be familiar and comply with relevant Operational, Personnel, Health and Safety policies and procedures, including Fire, COSHH, No Smoking and Alcohol
• Maintain a safe environment by adhering to local infection control policies
