An exciting opportunity has arisen to join a well-established children’s transition team at Colchester as Transition Nurse.

The role ensures that young patients and their families are well-supported during this critical phase, helping manage their medical needs, build confidence, and navigate the often challenging shift between healthcare systems.  The Nurse Specialist works closely with multidisciplinary teams to deliver individualised care plans, empowering young people to take control of their health and promoting better long term outcomes.

You will be offered opportunities to develop both clinically and professionally within a supportive environment and encouraged to participate in service developments. Clinical supervision and a supported induction period are provided.

Main duties of the job

  • To support the co-ordination of transition services and run nurse-led clinics for young people beginning / throughout their transition pathway.
  • To promote a young peoples focused approach to care in collaboration with all relevant health, social care and other agency professionals
  • To maintain a knowledge base that reflects contemporary issues in young                   peoples care / transitional care and to be able to select and apply the                             most appropriate management pathway recognising the growth and                             development of the child into and through adulthood
  • To act as an advisor to both children and adult health care staff on                                   matters relating to transitional care promoting the development of                               clinically effective care that is seamless between settings.
  • To act independently/autonomously within the team to manage the caseload, guided by best practices, trust policies and guidelines.
  • Use expert professional judgement, analytical and problem solving skills (evidence based) in the surveillance and recognition of changes in patient’s condition, referring changes to the clinicians as necessary
