The Children’s Community Learning Disability team are expanding and have an exciting opportunity for a creative and innovative Specialist Nurse who has experience of working with children who have a learning disability and their families. In the role you will have the opportunity to work alongside an experienced group of Learning Disability Nurses, Psychologists, Occupational Therapist, SALT and Psychiatrist.

You will be a member of the multi-disciplinary team providing assessment and intervention to Children and Young People with moderate to severe learning disabilities within the Children and Families Care Group, and may be required to work in a variety of settings including community settings, for example clinics, schools and family’s homes.

You will work directly with children and young people with learning disabilities and their families, carrying out assessments (including risk management) and effective treatment and/or interventions, with the aim of helping children and young people participate in normal family and community activities and to facilitate positive outcomes of care in relation to young people’s needs.

Main duties of the job

  • Take responsibility for the management of a complex caseload, which includes, specialist assessment (within service specification), care planning in conjunction with service user and carer, evaluation of individual treatment plans for CYP with learning disabilities.
  • Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy.
  • Assess patient's individual care needs, develop, implement and evaluate programmes of care.
  • Be responsible for the assessment of mental health and/or medical risk factors relating to children and young people, including co-morbidity (e.g. suicidality, self-harming behaviours and medical risk associated with mental health issues)
  • To undertake physical health monitoring as appropriate i.e height, weight, blood pressure etc and be responsible for identifying appropriate action to be taken
  • Deliver a range of interventions in accordance with the agreed plan of care and Pathway guidance.
  • Complete and contribute robust and dynamic risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve and offer optimal quality of life and health outcomes
  • Maintain an effective reporting system by observing and reporting  verbally and in writing on patient conditions.

In MPFT Children and Families Mental Health Function, we work with young people who have a wide range of needs using an inclusive, person-centered approach. Our model places our young people’s needs at the heart of what we offer to ensure that they are getting the best care possible. The Children’s Learning Disability Team offers: support and intervention for young people with emotional and physical healthcare needs who maybe at risk of placement breakdown or hospital admission.

You will have strong professional links through our Senior Clinical Nursing roles and through a supportive professional leadership structure sat within a ‘children and families care group’. Our Team regularly provides placements to Trainee Nurses
