CCN, Children's Community Nursing Team.
An exciting opportunity has arisen for an experienced and motivated Community Children’s Nurse to join our Community Children’s Nursing Service on a one-year fixed-term contract to cover maternity leave.
As an integrated organisation, we are uniquely positioned to provide seamless care across hospital and community settings. This role offers the opportunity to play a key part in that transformation and contribute to shaping the future of community children’s services.
You will provide expert clinical care and case management for children and young people aged 0–19 years with acute, chronic and complex health needs, supporting them to remain safely at home wherever possible and reducing avoidable hospital admissions through coordinated, proactive care planning. We are seeking an individual who is passionate about delivering high-quality, child-centred care in the community and who is committed to proactively optimising health outcomes for children and young people. In return, you will have the opportunity to further develop your leadership skills within a supportive senior leadership team and be part of a service that is actively redefining community healthcare delivery.
