ACCOUNTABLE TO

Head of Service; Community Paediatrics, Community Children’s Nursing and Specialist Services.

JOB SUMMARY

To provide clinical leadership and support for a mixed skill Community Children’s Nursing Team, ensuring high quality service provision to children with nursing needs at home and in other community settings, and their families. The post holder will fulfil general management responsibilities for a designated Community Children’s Nursing Team including nurses, Health Care Workers and administrative staff.

Maintain clinical competence and deliver clinical care to children and young people.

Promote and monitor safe, effective child and family centred care.

Support the Lead Nurse to ensure delivery and development of service based on assessed need and in line with strategic direction.

Support the coordination of discharges and transitions for children with complex nursing needs to include initial hospital discharge, and subsequent transitions between hospital, home, and school and between different services and directorates, ensuring excellent communication, robust interagency working and timely discharge/transition arrangements.

Develop partnership approaches with Children’s Disability Social Work teams, Medical and Allied Health Professionals and pursue networking opportunities across professional, organisational boundaries and with voluntary bodies.

KEY DUTIES / RESPONSIBILITIES

Professional Responsibilities

Comply with NMC Code of Professional Conduct.

Ensure practice reflects the standards set in the NMC Code of Professional Conduct: Professional Standards of Practice and Behaviour for Nurses and Midwives.

Comply with NMC requirements to ensure continuing professional development.

Comply with Regional Safeguarding Policies and Procedures and ensure Trust Policies and Procedures for safeguarding are adhered to.

Leadership and People Management

Provide day to day management of a mixed skilled Community Children’s Nursing Team.

Work in partnership with staff and support the Lead Nurse with the implementation of the Trust’s Nursing and Midwifery Strategy.

Ensure effective communication within the team.

Promote good staff relationships and team morale through staff engagement, effective feedback, recognition and appreciation.

Observe for signs of ill health or stress in staff and take appropriate action.

Ensure prompt and efficient procedures for monitoring and management of sick leave are implemented according to Trust policy.

Ensure systems are in place to provide managerial cover for the Community Children’s Nursing Team during periods of e.g. annual leave or study leave.

Participate in selection and interviewing panels as required.

Participate in disciplinary/ grievance/capability procedures as per Trust guidelines.

Service Delivery

Plan and implement duty rotas and annual leave arrangements within existing resources, to ensure an appropriate skill mix to deliver care based on assessed need.

Undertake ongoing caseload review and ensure fair and equitable caseload allocation.

Oversee and support staff to manage and review packages of care and short break provision.

Support the co-ordination and facilitation of Multi-Disciplinary Team meetings required to assess, plan, implement and review care of children on the Community Children’s Nursing caseload.

Develop robust communication channels ensuring a contact point is available for all staff from the multiple agencies involved in the child’s care.

Quality and Governance

Engage in the Trust’s Strategy for Clinical and Social Governance including participation in standard setting and audit on a uni-disciplinary and multi-disciplinary basis to measure quality and effectiveness of services.

Ensure safe, effective, person centred delegation of nursing tasks and duties.

Identify any early concerns or dissatisfactions that children/families may be experiencing; be proactive in addressing these and ensure appropriate responses and necessary changes are implemented.

Comply with Trust policy in regard to all incidents/complaints and report and record appropriately.

Support the Lead Nurse in the investigation of complaints and formulation of reports.

Take part in, and ensure all team members engage in clinical supervision, staff appraisal and Personal Development Review.

Identify and manage risks and report areas of high risk to Lead Nurse.

Adhere to, and ensure staff are updated and informed of all relevant policies and procedures.

Participate in policy development and review.

Collaborative Working

Participate and promote multidisciplinary working within the team and across all programmes of care.

Liaise with the multidisciplinary team, voluntary agencies and all other relevant personnel on a regular basis to promote and ensure effective communication is maintained and good work practices are facilitated.

In collaboration with the Lead Nurse facilitate service development and contribute to the Business Planning and contracting processes within the Trust.

Clinical Responsibilities

Maintain clinical competence in all aspects of the Community Children’s Nursing role.

Undertake clinical visits as required.

Support Community Children’s Nursing on call rota.

Provide clinical advice and support to staff.

Educational Responsibilities

Identify own educational needs with the Lead Nurse.

Actively encourage the professional and career development of nurses and Health Care Workers.

Undertake training needs analysis to identify training needs to meet existing and developing services.

Facilitate education and training of team members, Health Care Workers and staff in education and other areas as required, ensuring the robust application of competency-based process.

Initiate and facilitate research and quality improvement projects.

Ensure students are facilitated to avail of the range of learning opportunities available, appropriate to the needs of the student and with the consent of the child, young person and parents.

Ensure an induction programme is provided for all new staff and preceptorship programme for newly qualified nurses.

Finance and Resource Management

Deliver safe and effective care within allocated resources.

Adhere to financial Policies and Procedures and Trust financial controls

Contribute to budgetary management, including the monitoring of effectiveness of resources and value for money eg, equipment, disposables etc.

Information Management

Ensure accurate and timely completion by the team of all data for information systems.
