Job overview

An exciting and rewarding opportunity has arisen for a Case Coordinator to join the team in the West Integrated Transfer of Care Hub (iToCH). Currently based in Helston community Hospital. There would be an expectation for you to hot desk and shadow staff during your induction at Camborne Health Office and Penwith Health Centre. You will play an important role in supporting clinical staff with admission avoidance, arranging community support, case managing non-urgent referrals and facilitating safe and timely discharge from the acute and community hospitals for patients.

You will help provide a seamless service for the people we serve which will include cross boundary and collaborative working with the multi-disciplinary team, along with colleagues in Adult Social Care and other outside agencies in the community setting.

This is an ideal opportunity for an enthusiastic and dynamic individual. You will need to have good interpersonal skills, IT skills and possess excellent communication and organisational skills.

We will offer a supportive working environment in which to continue to develop your skills and knowledge in patient flow through a structured in-service training programme, appraisal, and supervision systems.

This is a permanent, full-time post, working 37.5 hours per week to include weekends and bank holidays on a rota basis, as the service is available 8-8.

Main duties of the job

  • The aim of the post is to achieve a collaborative approach to progression of community referrals and at times hospital discharges. To  ensure that patients receive quality  care by the right service in a timely manner .
  • The Case Coordinator will ensure that patient’s needs are addressed, based on the referral, and coordinate their care in the most timely and effective way, utilising all the available resources and capacity.
  • The Case Coordinator will obtain information from health and social care  professionals, patients and family or carers (as appropriate)  to track the progress of patients through the system and ensure quality care delivery.
  • The post holder will maintain communications with the team or referrer of any problems regarding timing of interventions, including equipment ordering/delivery and organisation of on-going care.
  • The Case Coordinator will liaise with other agencies and departments, to provide efficient and effective communication to ensure efficient care delivery.
