JOB DESCRIPTION

JOB TITLE: CARERS DEVELOPMENT WORKER

BAND: BAND 4

CONTRACTED HRS 15 HOURS PER WEEK

DIRECTORATE: PRIMARY CARE AND OLDER PEOPLE

LOCATION: ROSSABBEY OFFICES, ABBEY STREET, DERRY/LONDONDERRY

REPORTS TO: CARERS SUPPORT CO-ORDINATOR

ACCOUNTABLE TO: ASSISTANT DIRECTOR OF CARE & ACCOMMODATION

JOB SUMMARY

To work with the Western Trust Carers Support Coordinator to maintain and develop services to unpaid carers across the Western Trust area by supporting carers groups, inputting data into the Trust’s Carer’s database; maintaining links with Trust staff and Voluntary organisations, the planning of, and communication about, carer health and pamper events; and updating information for unpaid carers. This will involve evening and weekend work.

KEY DUTIES / RESPONSIBILITIES

1. To implement key actions of the Carers Strategy as directed by the Carers Coordinator.

2. To promote the Trust’s Carers and Direct Payments scheme.

3. To input data onto the Western Trust Carers I.T. systems.

4. To analyse Carers Assessment data collection and report to the Carers co-ordinator on actions.

5. To develop and update a range of information guides for informal/unpaid Carers across programmes of care.

6. To provide information to, and maintaining links with: unpaid carers, carer support groups, Trust staff and voluntary organisations. Provide specialised information to individual informal/unpaid carers on a range of issues including services, benefits and employment issues.

7. To link with communications department to ensure carers information is kept up-to-date on the Trust website and Facebook page.

8. Explore innovative and creative ways to keep individual unpaid carers informed of developments within the Trust.

9. To plan, arrange and deliver a variety of events for unpaid carers, for groups ranging from 10 -100 carers: e.g. Information events, health fairs, social & well-being events, etc. as directed by the Carers Coordinator.

10. To work with Carers Groups and individual unpaid carers to develop capacity and links in order to engage with Public and Personal Involvement, which will contribute to service development within the Western Trust.

11. To develop a carer network which will feed into the Western Trust’s Carers steering Group.

12. To develop and facilitate a local carer workers network.

13. To administrate the Carers Cash grant scheme, including authorising signature, recording on spreadsheet and forwarding payment request to finance.

14. To develop a range of training programmes for unpaid carers.

15. To answer and deal with queries from unpaid carers by phone, electronically and in person. To deal with Carers who display challenging behaviour in a sensitive way, and take appropriate actions to support the carer to a point of resolution.

16. To answer and deal with queries from Trust staff and others.

17. To order stock as required

18. To allocate work to and supervise band 3 administrator.

19. To attend meetings as appropriate.

20. To represent the Trust at Carer Support Group meetings as required.

21. To ensure a confidential service is maintained.

22. To collate information as required e.g. unmet need identified through Carers Assessment and Carers training needs

23. To deal with correspondence as required.

24. To assist the Trust’s Carers Support Co-ordinator with other duties as required

25. To comply with relevant Trust Policies and Procedures

GENERAL DUTIES
