Thank you for your interest in this Band 6 Mental Health Practitioner position within our Ealing Community Rehabilitation Service (CRS). This is a full-time and permanent position and attracts 15% Outer London weighting. We welcome applications from Registered Mental Health/LD Nurses (registered with the NMC) and qualified Social Workers (registered with Social Work England) who are interested in working in complex psychosis in the community.

Mental health practitioners play a key role in dissolving the barriers that can exist between mental health and physical health, social care, voluntary and community social enterprise organisations and local communities, and primary and secondary care, to deliver integrated, personalised, place-based and well-coordinated care for adults – moving towards a ‘whole system’ of mental health care, which will provide timely access to support, care and treatment.

We need qualified, experienced, skilled and professionally registered practitioners, to support people with mental health needs to access mental health care when and where they need it, and to be able to move through the system easily.

What we are looking for are highly motivated and enthusiastic practitioners with a passion for psychosocial rehabilitation who are excited by service development. You will need experience at band 5 (or equivalent) and of working in the community to be able to step up to this band 6 role.

Main duties of the job

Depending on your professional background, this role may include clinical assessment and evidence-based interventions, coordination of mental and physical healthcare, relapse prevention and recovery-focused support, alongside the psycho-social interventions that help people manage their problems and move towards individualised recovery on their own terms – surrounded by their families, carers and social networks, and supported in their local community.

You will be an autonomous practitioner who will carry continuing responsibility for a defined caseload of service users experiencing complex psychosis.

You will support individuals to be able to contribute to and be participants in the communities that sustain them. A key part of the role will be liaison with Local Authority and ICB colleagues to ensure that any out-of-area and supported accommodation placements are suitable for the needs of the service user and that people are supported in the least restrictive environment possible.
