Job overview
We are looking for a dynamic, pro-active, and experienced physiotherapist or cardiac exercise specialist with a strong background in leading the delivery of multi-modal cardiac rehabilitation programme within a community setting.
You will play an active and proactive role towards the continuous development of our exercise service provisions to help ensure delivery of high-quality cardiac rehabilitation programme that our service can be truly proud of. This includes facilitating home-based exercise programs for patients with heart failure and exploring the logistics and viability of virtual exercise programs with advice and guidance provisions.
You will be working with our small but effective and committed team of cardiac rehabilitation specialists and will be supported by our clinical team lead towards your ongoing development within this job role.
Main duties of the job
This role has clinical, professional, clinical governance, communication, teaching and mentoring, and research and audit remits.
- Lead, co-ordinate, and develop exercise assessments and supervised exercise sessions within the Cardiac Rehabilitation Programme, ensuring safe and effective delivery of care.
- Assess, prescribe, monitor, and progress individualised exercise programmes in line with BACPR guidelines, clinical assessment findings, and patient needs.
- Support patients to achieve lifestyle and behaviour change through education, health promotion, goal setting, and ongoing motivation.
- Contribute to the planning and delivery of patient education sessions, discharge planning, and the promotion of long-term physical activity opportunities within the community.
- Maintain accurate clinical records, manage risk appropriately, and work collaboratively with the multidisciplinary Cardiac Rehabilitation Team to deliver high quality patient centred care.

Essential
- Experience of team working
- Experience of prescribing and delivering exercise programmes/rehab
- Experience of planning, leading and evaluating exercise sessions for specialist populations
- Evidence of basic understanding of common medical conditions encountered on a typical exercise referral scheme and the necessary programming adaptations
- Evidence of health related benefits of regular physical activity for healthy adults and special populations
Desirable
- Experience in delivering cardiac rehabilitation
- Experience of prescribing exercise to patients with cardiac conditions/at high risk of a cardiac event
Skills and Knowledge
Essential
- Proven planning and organisational skills
- Proven ability to prescribe and deliver safe exercise to patients with CVD
- Proven ability to organise, monitor and check work to ensure no element of a task is left unfinished
Desirable
- Experience/understanding of Cardiac Rehab Rehabilitation
- Comprehensive knowledge of CVD and its implications for excise prescriptions
Education/Qualifications
Essential
- HCPC Registered Physiotherapist or Clinical Exercise Specialist with clinical science graduate degree level qualification
- BACPR Phase IV Qualification
Desirable
- Other recognised exercise qualifications.
- Experience in cardiology or respiratory care
