
Job overview
The Skin Cancer Support Worker (CSW) for Stratified Follow up will form an integral part of the multidisciplinary team approach in caring for the physical, social and psychological needs of patients diagnosed with cancer throughout the care treatment pathway and after care support.
The CSW will work in close unison with the Clinical Nurse Specialists (CNSs) to ensure the coordination and management of patients at all points of their treatment but in particular the this role will act as a point of contact for patients who will have been prepared by their clinical team for Stratified Follow Up and signpost patients to support their self- management.
The CSW will work independently within the remit of the role but under the direction, guidance and supervision of the Personalised Care Manager and Clinical Nurse Specialists.
