Job Description Summary (Main duties and responsibilities) Working within a multi-disciplinary team to assess the needs of CYP alongside registered Mental Health Nurses, Social Workers, an Occupational Therapist, Psychotherapist, Psychologist and Peer Support. Working with young people and their families/ carers to develop holistic care plans / treatment interventions Working with other agencies in a collaborative way to manage risk Working within a team who prioritise compassion, adaptability and creativity Undertake routine and profession specific outcomes measures Completion of social circumstance reports, individualised care plans and risk assessments Work in conjunction with other community and health professionals to deliver joined up care pathway to meet the specific need of the child/ young person and their family. Contribute to audits and quality improvement activities and actively participate in service development.
